How to Obtain a Passport
(for first-time applicants)

If you are obtaining a passport for the first time, you must apply in person.

Minor children and both parents must be present when an application is made for the child.  A minor is anyone age 15 years and younger.

A Statement of Consent or Special Circumstances may be completed in situations where only one parent is available.

Do NOT sign the DS-11 application form until a Passport Acceptance Agent instructs you to do so.

Required Identification

1.  You must provide proof of U.S. citizenship: (one of the following)

  • Certified Copy of Birth Certificate
     
                       or

  • Original Naturalization Certificate
     
                        or

  • Most recent previous U.S. passport

2.  You must also provide current identification

  • Driver's license with signature and photograph

    Parent's identification can be presented on behalf of
    a minor under age 18, when the minor does not possess
    his or her own identification.

You will need two separate forms of payment:

  • Check or money order is payable to the U. S. Department of State for the Passport Fee, Execution Fee and Return Express Mail fee.

  • Check, money order or cash may be used as payment to the City of Richfield for the Execution Fee, Pictures and Outgoing Express Mail.

The average time to receive a passport is approximately six weeks.

* For expedited service the average time to receive a passport is approximately two weeks.

** If expediting and overnight express mailing the application to the Passport Agency, you must arrive by 4:00 p.m.

Passports for adults are valid for 10 years and child passports are valid for 5 years. 

Passport Photos

Applicants may have passport photos taken at the time of applying, or submit their own photos as long as they meet the following guidelines.

  • 2x2 inches in size
  • Identical
  • Taken within the past 6 months, showing current appearance
  • Color or black and white
  • Full face, front view with a plain white or off-white background
  • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
  • Taken in normal street attire

U.S. Passport Card applications accepted beginning February 1, 2008!



Applications for a U.S. Passport Card, used to enter the U.S. by land and/or sea from Canada, Mexico, the Caribbean and Bermuda, will be accepted beginning February 1, 2008.  The card cannot be used for travel
by air.

First-time applicants will pay a $45.00 fee for adult cards, $35.00 fee for minor cards for children age 15
years and younger.

If an applicant already has a U.S. Passport, the cost of the Passport Card will be $20.00.

However, we need to point out that the card will not be issued until late Spring.
